Welcome To OMG Physical Therapy

Orthopaedic Medical Group of Tampa Bay is proud to have added physical therapy services for our patients. OMG Physical therapy provides quality rehabilitation with services using licensed physical therapists, physical therapy assistants, and athletic trainers. Our expert staff uses up to date rehab techniques and protocols based on peer-reviewed protocols and the latest scientific information

Our physicians oversee our rehabilitation protocols and ensure open lines of communication between the therapists, athletic trainers and physicians. Many of our athletic trainers work along side both the therapist and physician.

If you would like to make, cancel or change and appointment, please click the Online Appointments Tab on top of the page. After filling out the form completely, we will have someone from our office call you back and complete the scheduling process. This will be done within 1-2 business days.

Be sure to bring your insurance card, driver’s license or identification card and physical therapy script to you initial appointment.

Please do not use this form if you have an urgent medical problem. The offices & scheduling departments are closed on weekends. If you have an emergency, please call 911 or visit the nearest emergency room.
